To help you keep life in focus, vision coverage through EyeMed provides benefits for eye exams and vision correction treatment.
OVERVIEW
You can enroll in vision coverage as a new hire, during Open Enrollment, or if you have a Life Event. To see your Monthly Rate and enroll, log in to the UKG.
Medical-Dental-Vision Opt Out
You may choose to waive East West Bank coverage if you have access to alternative coverage (e.g. through a spouse’s employer). To waive medical coverage, you must provide proof (e.g. Medical ID card). Please refer to How to Waive Coverage for information.
The vision plan, provided through EyeMed, offers you the freedom to see any provider you choose, but the plan generally pays better benefits when you receive care from providers in the EyeMed network.
Take advantage of additional benefits by seeing a PLUS provider. You can enjoy a $0 copay for an exam and an extra $50 allowance for frames or contacts by seeing a PLUS provider.
Look for the PLUS provider icon on the provider locator to easily find a participating provider.
PLUS providers include Lens Craters (in-store and online), Target Optical (in-store and online), Pearle Vision and selected independent Providers.
If you go to an out-of-network provider, you will be reimbursed for exams and eyewear according to a schedule of benefits. Need to submit an out-of-network claim? Get the claim form here.
